THE CHRONIC CONCEPT arises from the study and evaluation of the specific needs of the home care of chronic patients:

• Information capture at home enhances the potential benefits of both home hospitalisation and home support in selected subgroups of patients with severe chronic disorders.

• Information technologies play an important role to efficiently handle the complexities of information management in integrated health care delivery systems

The CHRONIC SOLUTION is contributing to:

• Develop an information environment for the care of chronic patients,
• Define a new European chronic health care model,
• Offer an efficient alternative to conventional institutional care, by means of:
• Improving the quality of life of the patients,
• Defining an optimum working space for all the professionals involved,
• Reducing the immense costs associated to the care of chronic patients in any country of the EU, getting an optimum benefit of the technology and the common space of the Information Society.

The final aim of the CHRONIC PROJECT is to improve the quality of life of the patient by getting an optimum benefit of the emerging Information Society technologies.
